Immunization - for adults and children

School Immunizations

Who should get inactivated influenza vaccine? People 6 months of age and older at risk for getting a serious case of influenza or influenza complications, and people in close contact with them (including all household members) should get the vaccine.  For more information click on the link.

Mommy & Me - A program for Passport insurance members which provides a prenatal and post-partum home visit by a registered nurse providing information and support.

Nutritional Services - Individual counseling by appointment and Nutritional Education classes are provided by a Registered Dietitian.  Topics may include diabetes, cholesterol, weight control or other specific health concerns.  Nutrition Presentations to community, church, and school groups may be requested.  Community programs include topics such as Eat More Fruits and Vegetables and the 1% or Less Milk.

Upcoming Diabetes Support Group Meetings

WIC (Women, Infants & Children) - A supplemental food program for those meeting both income and nutritional eligibility guidelines.   Nutrition counseling and food vouchers
 for:
 • pregnant and postpartum women
 • breast-feeding women
 • Infants
 • children at nutritional risk up to age 5

 Family Planning
 • Annual exams
 • Preconceptual counseling
 • Birth control methods
 • Pregnancy testing

 
Well Child Exams - Periodic screenings from birth to 21 years.  The patient must qualify financially for this service.

Tuberculosis - Testing, treatment and follow-up

STD and HIV/AIDS - Testing and treatment for most sexually transmitted diseases.  HIV counseling and testing. Referral services.  All services are CONFIDENTIAL.

EPSDT - Early Periodic Screening, Diagnosis and Treatment Services - This program is for children from birth to age 21 who are eligible for Medicaid.  Under the EPSDT program, children are checked for medical problems early.  There are specific tests and treatments as they grow older.  The areas of health care that are checked include:  preventive check-ups; growth and development assessments; vision; hearing; teeth; immunizations; laboratory tests.
